ASCMI Form Available in SmartCare; CCC Is Deactivated
The CCC form has been deactivated in SmartCare as of April 1. The new Authorization to Share Confidential Member Information (ASCMI) Form is available for immediate use. Counties should begin using the ASCMI Form to document a client’s consent decision regarding the sharing of protected health information.
While the ASCMI Non-AB 133 Form in SmartCare remains the recommended version for use, additional scanned document types have been added to support documentation of other ASCMI DHCS forms that may be received. These include ASCMI AB-133 (scanned) and ASCMI Revocation (scanned).

Updated Invoice Generator Versions Available
New versions of the Invoice Generators are available for download from the CalMHSA EHR Knowledge Base. Key updates include:
- Enhancements to support upcoming changes to the Contract Provider Charge Export
- Optional inclusion of specific client demographics needed for BHSA reporting (in an upcoming release)
- Removal of the NTP_Commercial and OtherRates-Commercial tabs across all fiscal years, as they are not used for invoicing
- Updates to the NTP Rates tab including removing parentheses from the Narcan procedure name (Naloxone HCL – 2 pack Narcan) to ensure accurate invoicing
Updated Invoice Reconciliation Tool Available
A new version of the Invoice Reconciliation Tool is available on the CalMHSA EHR Knowledge Base.
This update ensures that the Invoice Reconciliation process can be used to verify invoices that include the most recent updates to the Invoice Generators.
