CalMHSA TADT Report

Report Description

This report is designed to display TADT records from the following TADT forms in SmartCare – MHP Psychiatric, MHP Non-Psychiatric, DMC-ODS Outpatient, DMC-ODS Opioid.  The report will select records based on the Effective Date of the respective TADT document within the date range entered by the user in the Start and End Date parameters.
