CalMHSA Client Medical Face Sheet

Report Description – This report is designed to display client medical related data and is aimed at medical users.   The user can select one or more categories in the Show parameter for different types of data such as allergies, medications, labs, etc.  The report is client based so the user must choose a client prior to launching the report and the report will enforce CDAG security where applicable.

Used to populate the diagnosis section and only pulls in the last Diagnosis document PER non discharged Program and bring in all relevant ICD10 codes  (CDAG secure)

   For the Problem section, pulls in all problems currently open in non discharged programs  (CDAG secure)

   Then groups by ICD10 code and brings in

     1- the most recent date (effective for dx or start for problem)

     2- a list of the source (dx or problem) and the programs it is active in
