Overview
The ASCMI Completion Report was built to answer the question, “Which clients that should have a completed ASCMI have one?” This question is about implementation, and ensuring that as many clients as possible within a program have an ASCMI on file.
This report will use the program parameter to pull any clients who have an enrollment in that program. The system then reviews that client’s record and determines if an ASCMI exists for that client, regardless of what program that ASCMI was done under (within CDAG rules). So if you’re attempting to ensure that all clients in Program A have an ASCMI, but Client X had an ASCMI completed by Program B, you’ll be able to see that Client X has an ASCMI on file from Program B, and Program A doesn’t need to do another one.
The report only pulls the most recent ASCMI on file, as a newer ASCMI will supersede an older ASCMI. The report prioritizes ASCMIs that the client or guardian has signed, as that is considered complete. If no ASCMIs exist on file for a client where the client/guardian has signed, the report will find the most recent ASCMI that a staff member has signed. This indicates that an ASCMI was started with the intent for the client/guardian to sign, but the signature hasn’t yet been captured. If there are no ASCMIs on file where the staff member has signed, but an ASCMI is in progress, the report will provide that information. If there are no ASCMIs on file anywhere, in progress or otherwise, the report will leave the ASCMI fields blank, but still return the client, to indicate that this client is expected to have an ASCMI but none exists.
The report should provide enough information to determine the status of the ASCMI. It includes the author of the document (the staff who at least started the ASCMI). It also includes the staff signature date. If the staff signature date field is blank, that indicates the staff author has not signed the document yet. The report then provides information about the client’s signature date. If this field is blank, it indicates that the client/guardian hasn’t signed the document. It also includes the person who signed (client or guardian) and their relationship to the client. That indicates whether the client signed themselves or if a guardian signed on their behalf.
For records that only return a client’s name, this indicates that the report is not finding any ASCMI on file, but this client is enrolled in one of the programs listed in the parameters. This report abides by CDAG rules, so if an ASCMI exists for a program outside of a user’s CDAG and the client has not authorized CDAG rules to drop, this report will show that no ASCMI exists. This ensures 42 CFR Part 2 privacy rules are followed.
Date Range Parameters
In the parameters, we have a “Date Range Type” field. This defaults to “Active Enrollment”. When you enter a date range, the system will look for any clients who had a program status of “Enrolled” at any time during the date range. For example, this includes clients who were enrolled prior to the date range who are still enrolled. This also includes clients who were discharged during the date range, so long as they had a status of “Enrolled” at some time during the date range.
The other Date Range Type is “Enrolled during the date range”. If you use this type, then the system will only return clients who have an Enrolled Date within the date range specified.
Sort Order and Export
This report sorts by program first, then by client. If you want to sort in alternative methods, we recommend exporting the report results to Excel.
