History:
AB 2242 made amendments and additions to the Lanterman-Peteris-Short Act (LPS) Act.
The Care Coordination Plan is required as part of BHIN –24-039 when discharging individuals who experienced temporary holds under W&I Code section 5152 or a conservatorship. This requirement applies to county behavioral health departments and LPS-designated facilities.
Each county behavioral health department shall ensure that a care coordination plan that ensures continuity of services and care in the community for all individuals exiting holds or a conservatorship is established.
Considerations
As of November 18, 2024 , DHCS provided a template for model care coordination plan. It can be used by facilities as a printable pdf.
In the near future, CalMHSA plans to create an embedded editable version, but in the meanwhile, many of the key data fields can be found in CalMHSA Aftercare Discharge/Transfer Summaries Report. For now, your facility can use the template provided above separately and/or if your facility may want to create Keyphrases as prompts and use either CalMHSA Discharge and/or Transfer Summary to add any additional data as part of the free textboxes, this information will then be pushed to the CalMHSA Aftercare Discharge/Transfer Summaries Report.
