ASCMI Form Training Webinar

The Authorization to Share Confidential Member Information (ASCMI) Initiative is a statewide effort to promote and standardize the exchange of Clients’ sensitive information, including certain physical health, behavioral health, and social services information (HSSI), among Care Partners such as providers, health plans, county agencies, and social services organizations.

The ASCMI form was developed by DHCS and outlines the information a client agrees or does not agree to share, as well as how it may be shared and used with other partners. The form meets compliance with applicable state and federal law.

More information can be found on the DHCS website.

Please contact CalMHSA for questions related to the ASCMI form in SmartCare at [email protected].
