Medication History pulls data for that specific client from the Pharmacy Benefit Manager, SureScripts, and provides a comprehensive list of prescriptions that have been filled for the customer (from any prescribers within or outside of the organization). This data is held by the national Pharmacy Benefit Manager and that is the source of this information. This tab will populate with all the client’s medication in the past two years, up to 100 medications.
Note: This is referencing Medication History tab, not Medication History button.
Aside from technical set up (e.g. SureScript connection), two factors are required for the medication history to populate, consent and medication visit.
Medication History Request Consent
In order for the “Medication History” functionality to work, the client must have a valid complete Medication History Request Consent document in the client record. This is required from SureScripts as part of the certification and functionality.
- With the client open, then click the Search icon.
- Type “Medication History Request Consent” into the search bar.
- You can search for any word within Medication History Request Consent and only characters are needed prior to showing options
- Select “Medication History Request Consent (Client)” from the search results.
- In the CDAG Program Enrollment window pop-up, click the drop down and click to select the appropriate program.
- Click OK to continue.
- The Medication History Request Consent document will open. Enter the start date and the end date, if applicable.
- Click Sign.
- This will create the PDF. Click the Plus icon to expand the document ribbon.
- The client is automatically added as a cosigner. Select the client and click Cosign.
- This brings up the signature window. Document the client’s signature and click Sign.
- This adds the client’s signature to the bottom of the document. To print this out for the client, click the Print icon.
Medication Visits
A process will gather medications from the Pharmacy Benefit Manager. The process will look for any clients that have a scheduled medication visit in 72 hours. Your System Administrator will setup any procedure codes that are considered Medication Visits on the back-end of the system.
