PDF Headers 

For documents in SmartCare, when they are signed, a PDF version of the document is created. This is managed by the creation of an RDL for each and every document that exists, which drives what the PDF looks like. Headers and footers are generally more difficult to manage, but Streamline has 4 data points that a system administrator can add to all documents as a feature in the header. These configuration keys manage whether these data points show up on the header or not. By default, CalMHSA does not have any of these on, as documents are often viewed online rather than printed, where pages can get separated and may have need of including client data points, such as name or client ID on each page in case of separation. Please note that as of the August 2025 MSP, these headers will also show on any scanned or uploaded document PDFs once added to a client’s chart.   • SetFormatToShowClientNameOnAllPagesOfPdf (Client Name format) 
  • ShowClientDOBOnAllPagesOfPdf (DOB visibility) 
  • SetDateValueToShowMedicaidOnAllPagesOfPDF (Medicaid ID logic) 
  • ShowEffectiveDateOnAllPagesOfPDF (Effective Date visibility) 

SetFormatToShowClientNameOnAllPagesOfPdf

Details: Determines if and how the client’s name shows in the header of PDFs, meaning it will show on all pages of a PDF.  

Allowed Values:  

  • “None” 
  • The client’s name will not be displayed in the header 
  • “LastNameFirstName” 
  • Will display the client’s name as “LastName, FirstName” 
  • “FirstNameLastName” 
  • Will display the client’s name as “FirstName LastName” 
  • “FirstNameMiddleInitialLastName” 
  • Will display the client’s name as “FirstName M. LastName” 

 

Default Value: “None”  

ShowClientDOBOnAllPagesOfPdf

Details: Determines if the client’s date of birth shows in the header of PDFs, meaning it will show on all pages of a PDF.  

Allowed Values:  

  • “No” 
  • The client’s DOB is not displayed in the header 
  • “Yes” 
  • Will display the client’s date of birth in MM/DD/YYYY format 

 

Default Value: “No”  

SetDateValueToShowMedicaidOnAllPagesOfPDF

Details: Determines if and how the client’s Medicaid ID number shows in the header of PDFs, meaning it will show on all pages of a PDF.  

Allowed Values:  

  • “None” 
  • Medicaid information is not displayed in the header. 
  • “CurrentDate” 
  • Displays the Medicaid ID from the active coverage plan, based on the current date 
  • “EffectiveDate” 
  • Displays the Medicaid ID from the active coverage plan, based on the document’s effective date 
  • “Anytime” 
  • Displays the Medicaid ID from the lowest COB coverage plan at any time 

 

Default Value: “None”  

Note: If the configuration key is set to include Medicaid ID information but no Medicaid ID exists for the client, the system will instead display “Medicaid: NA” 

ShowEffectiveDateOnAllPagesOfPDF

Details: Determines if the document’s effective date shows in the header of PDFs, meaning it will show on all pages of a PDF.  

Allowed Values:  

  • “No” 
  • The effective date of the document will not be included in the header 
  • “Yes” 
  • The effective date of the document will be included in the header, which means it will show on all pages of the PDF 

 

Default Value: “No”
