History
AAB 2242 made amendments and additions to the Lanterman-Peteris-Short Act (LPS) Act. DHCS published BHIN 24-039 on November 18, 2024 in response to these changes.
Summary of Requirements
When discharging a client from an LPS facility who is on an involuntary hold or is LPS conserved, there is a shared responsibility for that client between the county, the facility, and the health care payer, if different from the county. To ensure the client’s continuity of care, a Model Care Coordination Plan, or MCCP, must be created.
The MCCP must include:
- A detailed plan
- A scheduled first appointment with the health plan, the behavioral health plan, a primary care provider, or another appropriate provider to whom the person has been referred
DHCS includes an attachment sample MCCP template, but indicates that it is not a required DHCS form, and that facilities can create their own.
SmartCare Implementation
CalMHSA is developing an MCCP that works with the Shared Treatment Plan for ease of use. This is still in development.
In the meantime, CalMHSA recommends using the Discharge Summary or Transfer Summary document. These documents both include a section for either the Discharge Plan or the Transfer Plan, which have the description, “include follow-up appointments, methods of transfer, other transfer considerations”. These documents currently meet the requirements of the MCCP.
